DICOM conformance statement

This statement describes how Routes conforms to the DICOM standard (PS3.2 lays out what a conformance statement covers). It applies to every node of the current release; the central service has no DICOM interface of its own.

Routes is a DICOM router. It receives composite instances from modalities and other systems, keeps each one on disk until every destination it is routed to has it, and sends it on. Along the way it can convert the transfer syntax, edit or de-identify the instance, and answer storage commitment. It can also act for the archives behind it: it answers queries and retrievals by asking those archives, serves a modality worklist and accepts MPPS.

Routes does not display, create or interpret images, except for DICOMweb rendered images and thumbnails. It changes an instance only when you configure it to: with tag edits, de-identification or a transfer syntax conversion.

Per source and per destination services are offered only when that source or destination is set up for them.

Both are sent in every association request and acceptance, by every node, as a sender and as a receiver.

Each node is one application entity, the same on every node:

  • Its AE title is ROUTES by default (Configuration › Settings › AE title). Destinations can call as another AE title.
  • The ports it listens on are 11112 by default, plus TLS ports when they are configured. A node can shift all of its ports by an offset, which lets several nodes share one server.
  • The called AE title of an incoming association is not checked by default. A source can require particular called AE titles, and routing decisions can depend on them.
  • Receive and route. A sender stores instances (C-STORE). The node accepts each one once it is written to disk and queued for its destinations. Each destination’s queue then sends it (C-STORE) on its own association.
  • Storage commitment. A sender asks for storage commitment. The node answers once the instances are on its disk, or once every destination of their route has accepted them, as the source is set. The node can also request storage commitment from a destination after sending to it.
  • Query and retrieve. A sender queries (C-FIND) or retrieves (C-MOVE, C-GET). The node asks the archives its source is linked to and passes their answers on.
  • Worklist and MPPS. A modality queries the worklist (C-FIND), which the node answers from its copy of the orders. It reports procedure steps (N-CREATE, N-SET), and the node can forward them.
  • Prior studies. When a new study arrives, the node finds the patient’s earlier studies in an archive (C-FIND) and retrieves them (C-GET or C-MOVE) for the route’s destinations.
  • Verification. A node answers C-ECHO, and the console can test a destination with C-ECHO.

A node accepts an association only from a sender it has a use for:

  • a source accepts it (calling AE title, called AE title, IP address and port), for storage;
  • a source lets it query and retrieve, or query the worklist;
  • it is an archive delivering what the node asked it to move to itself;
  • or it is a destination delivering a storage commitment result.

A C-ECHO-only association is accepted from anyone, unless Restrict C-ECHO to known sources is on.

Services that the sender’s source does not allow have their presentation contexts refused (abstract syntax not supported), while the rest of the association is accepted.

The node accepts every storage SOP class in the table below, together with Verification, the Storage Commitment Push Model, and the query/retrieve, worklist and MPPS SOP classes its sources allow. With Accept unknown SOP classes on (the default) it also accepts SOP classes it does not know, such as private ones. It stores them unchanged and routes them like any other instance.

The node is a level 2 (full) storage SCP: it keeps every element, including private ones, unless an edit or de-identification you configure changes it. It does not check that an instance is valid against its IOD. Instances are accepted once they are on the node’s disk and queued, and the node takes responsibility for delivering them.

C-STORE status Meaning

0110 Processing failure: the instance could not be read, edited or queued

Refused instances can be kept in the node’s quarantine for diagnosis (Settings › Quarantine days; off by default).

Each destination’s queue sends instances with C-STORE, in priority order. It proposes the instance’s SOP class once for each transfer syntax in the destination’s list, in a separate presentation context, so the destination’s choice cannot change the order. The instance goes out in the first syntax the destination accepts, converted if need be (see Transfer syntaxes).

By default the node accepts any of the syntaxes below, choosing the sender’s first choice it knows. A source can narrow this to a list, choose by its own order rather than the sender’s, refuse what is not on the list, and set other rules for particular SOP classes (see What sources accept).

A destination receives the syntaxes in its list, most preferred first. By default the list is as received, then Explicit VR Little Endian, then Implicit VR Little Endian.

Lossy images are never compressed again with another lossy method. An image converted to a lossy syntax gets Lossy Image Compression 01, with its method and ratio, as the standard requires.

A source can offer the Storage Commitment Push Model (1.2.840.10008.1.20.1) to its senders. The node accepts N-ACTION (Request Storage Commitment, action type 1). It answers with N-EVENT-REPORT on a new association that it opens to the requester, once every referenced instance is committed:

  • When stored: on the node’s disk and queued; or
  • When delivered: accepted by every destination of its route (the default).

The result goes to the requester’s address, or to the reply host, port and AE title the source sets, optionally over TLS. Instances still not committed after the timeout (60 minutes by default) are reported as failed (event type 2).

A destination can be asked for storage commitment after instances are sent to it. The node sends N-ACTION on the sending association. It waits up to 10 seconds there for the N-EVENT-REPORT, then accepts it on a new association that the destination opens to the node. That association is recognized by the destination’s AE title, and only storage commitment is accepted on it. Instances the destination reports as failed can be sent again (up to twice). Those still not committed are recorded in the history as commit failed.

A source can let its senders query and retrieve (May query and retrieve). The node then accepts:

The node holds no images to query, so it acts as a proxy:

  • C-FIND is passed to the source’s archives in parallel. Their matches are returned as they come, each study, series or instance once, with the router as the Retrieve AE Title. Matching is the archives’ own; relational queries work if the archives support them.
  • C-MOVE retrieves from the first archive that has the instances. The move destination must be the called AE title of a configured destination; otherwise the status is A801. The instances are queued for that destination with its edits and de-identification, and sub-operations count as completed once they are queued.
  • C-GET retrieves from an archive through the router, and sends each instance back to the requester on the same association as it arrives. The requester must propose the storage SOP classes with the SCP role, and the node grants it.
  • Patient IDs are translated both ways when the source or the archive uses a patient ID map.
  • When no archive can be reached the status is C000 (unable to process). Every query and retrieval is recorded in the audit log.

Archives are destinations marked as archives. The node queries them with the Study Root model (C-FIND) and retrieves from them in one of two ways:

  • C-GET (Study Root, the default): the archive returns the instances on the same association, and the node takes the SCP role for them;
  • C-MOVE (Study Root): either to the router itself, which the archive must know as a move destination, or straight to the target, when patient IDs need no translation.

A source can let its senders query the worklist. The node accepts Modality Worklist Information Model – FIND (1.2.840.10008.5.1.4.31) and answers from its copy of the orders received over HL7 (see Modality worklist and MPPS for the matching keys). It answers when the central service cannot be reached. Answers are encoded in UTF-8 (ISO_IR 192).

A source with the worklist on accepts Modality Performed Procedure Step (1.2.840.10008.3.1.2.3.3) N-CREATE and N-SET, unless Accept MPPS is off. Procedure steps update the worklist item they refer to. Once the node has accepted them, it can forward them to other systems as an MPPS SCU.

The node passes every Specific Character Set (0008,0005) through unchanged. It decodes and re-encodes text only when an edit, de-identification or script changes it. Then it supports the default repertoire, ISO 8859 parts 1 to 9 and 15 (ISO_IR 100, 101, 109, 110, 144, 127, 126, 138, 148, 203), Thai (ISO_IR 166), Japanese (ISO_IR 13, ISO 2022 IR 87, ISO 2022 IR 159), Korean (ISO 2022 IR 149), Chinese (ISO 2022 IR 58, GB18030, GBK) and UTF-8 (ISO_IR 192). Code extensions that combine several repertoires use the first non-default one. Unknown terms are read as Latin-1, which keeps every byte intact.

  • DICOM TLS: nodes can listen on TLS ports and send to destinations over TLS, with TLS 1.2 or 1.3. They can require client certificates from senders (mutual authentication) and present their own to destinations. See DICOM TLS.
  • Who may connect is set by the sources: calling and called AE titles, IP address ranges and ports.
  • Audit: every instance received and delivered is recorded in the history, senders turned away are shown on the monitoring page, and queries and retrievals are recorded in the central audit log.

STOW-RS uploads are routed like C-STOREs; each sender has its own token. QIDO-RS and WADO-RS are proxied to the archives, as C-FIND and C-GET or C-MOVE are. Destinations can also be DICOMweb services, sent to with STOW-RS. See DICOMweb.
